Registration must be done in person at our office. You may stop by or register during a check-in for an appointment. Any of our front desk staff may assist you.

On the patient portal login page click Forgot Password?, enter your patient portal username and click submit. Instructions for resetting your password will be emailed to you.

The patient portal uses encryption (security measures) to keep unauthorized persons from reading communications, information, or attachments. Secure messages and information can only be read by someone who knows the correct password or passphrase to log in to the portal site.

A link to the patient portal site is included with your welcome email. You may also visit this website www.uchcaz.org and click the Patient Portal Link found at the top-right corner of this page.

Yes. You may request a refill for a medication prescribed by one of the United Community Health Center’s providers. We are unable to provide refills for medications prescribed by other providers.

No. Due to potential HIPAA privacy concerns, we are not able to grant access to a child’s chart at this time.

No. Due to HIPAA privacy laws each user of the portal must have their own account using their own email address.
